The two most common failure modes I see are the mechanism jamming locked and the mechanism jamming open. Jamming locked means the handle will not drop down, or the key turns but nothing moves. You are inside and the door will not open, or you are outside and the same problem applies. This is a lockout, and it usually needs the mechanism stripped out to release the door before anything can be replaced.
Jamming open is less dramatic but arguably worse for security. The locking points retract and then will not shoot back out. The door closes but does not lock. People sometimes do not realise this has happened and go to bed thinking the door is secured when it is not.

Can I just replace the cylinder rather than the whole mechanism?
Sometimes, yes. If the cylinder is the only thing at fault, swapping it out is straightforward and much less involved than replacing the whole gearbox.
